“Personal Data” means data that allows someone to identify or contact you, including, for example, your name, address, telephone number, e-mail address, as well as any other nonpublic information about you that is associated with or linked to any of the foregoing data. “Anonymous Data” means data that is not associated with or linked to your Personal Data; Anonymous Data does not, by itself, permit the identification of individual persons. We collect Personal Data and Anonymous Data, as described below.

In this case, we may receive information from that SNS to make it easier for you to create an Account on the Site. Any information that we collect from your SNS account may depend on the privacy settings you have with that SNS, so please consult the SNS’s privacy and data practices.

The e-mail that is sent to your friends will come from your e-mail address so that your friends know that you want to invite them to visit the Site.
E. Information Collected From Address Book and Contact Importing
You may use our address book and contacts importer to upload your contacts from other services into CareSwitch. When you import your contacts, we may indefinitely store the contact information in our database, including your contacts' names, emails, phone numbers, locations, and relationships to you. We do not store any passwords you provide as part of the contact import process.
After we import your contacts, we will use this information to help us establish networks of contacts, document relationships, facilitate referrals, and improve our services. We may also use this information to show you and other users that you may know each other or have shared contacts. We rely upon you to obtain whatever consents from the relevant third parties that may be required by law to allow us to access and upload the third parties’ names, e-mail addresses, and other contact information. You or such a third party may contact us at email@example.com to request the removal of this information from our database.
F. Information Collected From Third Party Companies
We may receive Personal and/or Anonymous Data about you from companies that provide our Service by way of a cobranded or private-labeled website, companies that offer their products and/or services on our Site, and/or companies that otherwise collect such information. These third party companies may supply us with Personal Data. We may add this information to the information we have already collected from you via our Site in order to improve the Service we provide.
In general, Personal Data you submit to us is used either to respond to requests that you make, or to aid us in serving you better. We use your Personal Data in the following ways:

We often receive testimonials and comments from users who have had positive experiences with our Service. We occasionally publish such content. When we publish this content, we may identify our users by their first and last name and may also indicate their home city. We will share your feedback with your first name and last initial only. If we choose to post your first and last name along with your feedback, we will obtain your consent prior to posting you name with your feedback. We are not responsible for the personally identifiable information you choose to submit in these blogs and forums.
C. Creation Of Anonymous Data
We may create Anonymous Data records from Personal Data by excluding information (such as your name) that makes the data personally identifiable to you. We use this Anonymous Data to analyze request and usage patterns so that we may enhance the content of our Service and improve Site navigation. Certain portions of the information you provide to us may also be displayed in your Profile. As an essential element of the Service, most of the Personal Data you explicitly provide to us when you register or update your Profile is displayed on your Profile. In order for your Profile to be made public, you must go to your profile settings and then to profile visibility. By default, your Profile is not for public viewing. Your photos, posts, friends, and other content you post to the Site are also meant for public consumption. We may display this content on the Site and further distribute it to a wider audience through third party sites and services. Once displayed on publicly viewable web pages, that information can be collected and used by others. We cannot control who reads your postings or what other users may do with the information that you voluntarily post, so it is very important that you do not put Personal Data in your posts. Once you have posted information publicly, while you will still be able to edit and delete it on the Site, you will not be able to edit or delete such information cached, collected, and stored elsewhere by others (e.g., search engines).

C. De-linking Sns
If you decide at any time that you no longer wish to have your SNS account (e.g., Facebook) linked to your Account, then you may de-link the SNS account in the “preferences” section in your account settings. You may also manage the sharing of certain Personal Data with us when you connect with us through an SNS, such as through Facebook Connect. Please refer to the privacy settings of the SNS to determine how you may adjust our permissions and manage the interactivity between the Service and your social media account or mobile device.
D. Changing Or Deleting Your Personal Data
You may change any of your Personal Data in your Account by editing your profile within your Account or by sending an e-mail to us at firstname.lastname@example.org. You may request deletion of your Personal Data by us, and we will use commercially reasonable efforts to honor your request, but please note that we may be required to keep such information and not delete it (or to keep this information for a certain time, in which case we will comply with your deletion request only after we have fulfilled such requirements). When we delete any information, it will be deleted from the active database, but may remain in our archives. We may also retain your information for fraud or similar purposes.
CareSwitch is committed to protecting the security of your Personal Data. We use a variety of industry-standard security technologies and procedures to help protect your Personal Data from unauthorized access, use, or disclosure. We also require you to enter a password to access your Account information. Please do not disclose your Account password to unauthorized people. No method of transmission over the Internet, or method of electronic storage, is 100% secure, however. Therefore, while CareSwitch uses reasonable efforts to protect your Personal Data, CareSwitch cannot guarantee its absolute security.
CareSwitch may use and disclose your PHI in the following ways:
Treatment, Payment and Healthcare Operations. CareSwitch is permitted to use and disclose your PHI for purposes of (a) treatment, (b) payment and (c) healthcare operations. For example:
Treatment. CareSwitch may disclose your PHI to another physician or healthcare provider for purposes of a visit or in connection with the provision of follow-up treatment.
Payment. CareSwitch may use and disclose your PHI to your health insurer or health plan in connection with the processing and payment of claims and other charges.
Healthcare Operations. CareSwitch may use and disclose your PHI in connection with its healthcare operations, such as providing customer services and conducting quality review assessments. CareSwitch may engage third parties to provide various services for CareSwitch. If any such third party must have access to your PHI in order to perform its services, CareSwitch will require that third party to enter an agreement that binds the third party to the use and disclosure restrictions outlined in this Notice.
Authorization. CareSwitch is permitted to use and disclose your PHI upon your written authorization, to the extent such use or disclosure is consistent with your authorization. You may revoke any such authorization at any time.
As Required by Law. CareSwitch may use and disclose your PHI to the extent required by law.
The following categories describe unique circumstances in which CareSwitch may use or disclose your PHI:
Public Health Activities. CareSwitch may disclose your PHI to public health authorities or other governmental authorities for purposes including preventing and controlling disease, reporting child abuse or neglect, reporting domestic violence and reporting to the Food and Drug Administration regarding the quality, safety and effectiveness of a regulated product or activity. CareSwitch may, in certain circumstances disclose PHI to persons who have been exposed to a communicable disease or may otherwise be at risk of contracting or spreading a
disease or condition.
Workers’ Compensation. CareSwitch may disclose your PHI as authorized by, and to the extent necessary to comply with, workers’ compensation programs and other similar programs relating to work-related illnesses or injuries.
Health Oversight Activities. CareSwitch may disclose your PHI to a health oversight agency for authorized activities such as audits, investigations, inspections, licensing and disciplinary actions relating to the healthcare system or government benefit programs.
Judicial and Administrative Proceedings. CareSwitch may disclose your PHI, in certain circumstances, as permitted by applicable law, in response to an order from a court or administrative agency, or in response to a subpoena or
Law Enforcement. CareSwitch may, under certain circumstances, disclose your PHI to a law enforcement official,
such as for purposes of identifying or locating a suspect, fugitive, material witness or missing person.
Decedents. CareSwitch may, under certain circumstances, disclose PHI to coroners, medical examiners and funeral
directors for purposes such as identification, determining the cause of death and fulfilling duties relating to decedents.
Organ Procurement. CareSwitch may, under certain circumstances, use or disclose PHI for the purposes of organ
donation and transplantation.
Research. CareSwitch may, under certain circumstances, use or disclose PHI that is necessary for research purposes.
Threat to Health or Safety. CareSwitch may, under certain circumstances, use or disclose PHI if necessary to prevent or lessen a serious and imminent threat to the health or safety of a person or the public.
Specialized Government Functions. CareSwitch, may in certain situations, use and disclose PHI of persons who are, or were, in the Armed Forces for purposes such as ensuring proper execution of a military mission or determining entitlement to benefits. CareSwitch may also disclose PHI to federal officials for intelligence and national security purposes.
You have the following rights regarding the PHI maintained by CareSwitch:
Confidential Communication. You have the right to receive confidential communications of your PHI. You may request that CareSwitch communicate with you through alternate means or at an alternate location, and CareSwitch will accommodate your reasonable requests. You must submit your request in writing to CareSwitch.
Restrictions. You have the right to request restrictions on certain uses and disclosures of PHI for treatment, payment or healthcare operations. You also have the right to request that CareSwitch restrict its disclosures of PHI to only certain individuals involved in your care or the payment of your care. You must submit your request in writing to CareSwitch. CareSwitch is not required to comply with your request. However, if CareSwitch agrees to comply with your request, it will be bound by such agreement, except when otherwise required by law or in the event of an emergency.
Inspection and Copies. You have the right to inspect and copy your PHI. You must submit your request in writing to
CareSwitch. CareSwitch may impose a fee for the costs of copying, mailing, labor and supplies associated with your
request. CareSwitch may deny your request to inspect and/or copy your PHI in certain limited circumstances. If that occurs, CareSwitch will inform you of the reason for the denial, and you may request a review of the denial.
Amendment. You have a right to request that CareSwitch amend your PHI if you believe it is incorrect or incomplete, and you may request an amendment for as long as the information is maintained by CareSwitch. You must submit your request in writing to CareSwitch and provide a reason to support the requested amendment. CareSwitch may, under certain circumstances, deny your request by sending you a written notice of denial. If CareSwitch denies your request, you will be permitted to submit a statement of disagreement for inclusion in your records.
Accounting of Disclosures. You have a right to receive an accounting of all disclosures CareSwitch has made of your PHI. However, that right does not include disclosures made for treatment, payment or healthcare operations, disclosures made to you about your treatment, disclosures made pursuant to an authorization, and certain other disclosures. You must submit your request in writing to CareSwitch and you must specify the time period involved (which must be for a period of time less than six years from the date of the disclosure). Your first accounting will be free of charge. However, CareSwitch may charge you for the costs involved in fulfilling any additional request made within a period of 12 months. CareSwitch will inform you of such costs in advance, so that you may withdraw or modify your request to save costs.
Breach Notification. You have the right to be notified in the event that CareSwitch (or a CareSwitch Business Associate) discovers a breach of unsecured PHI.
